Dear Colleague,

We are asking you to take part in the Induction Survey to give us your feedback about the Oregon Mentoring Program. This survey helps us to learn what’s working and what steps we need to take to ensure our program is responsive to you.

Your responses will be grouped with the answers of other participants in our program to help us tailor our support. Your name will not appear on your feedback, however, confidentiality is not guaranteed for program leads and mentors might be able to identify the writer of some open-ended comments, dependent upon the details provided by that respondent. In addition, beginning teachers, mentors and site administrators from several other programs will be taking this survey. The grouped responses from all programs will be used to inform the larger educational community about beginning teacher support. By taking this survey, you are able to contribute to the New Teacher Center’s ongoing research regarding induction programs and mentoring practices.

*Beginning Teachers: Please print this page and return it to your mentor to let her/him know that you have completed the survey!

*Please remember to continually SAVE your answers as you take the survey. The survey provides a “save” button to facilitate this. You will be timed out of the survey after 30 minutes and your answers on that page will not be saved automatically.
